New Patients
The first visit at a new doctor can be intimidating and can take a long time to fill out all the paperwork involved. Not to worry — your health, comfort and smile are our primary concerns. It is our purpose to provide the finest in dental care in a very relaxed, enjoyable atmosphere. It is our goal to have every patient leave the office happier and healthier than when they arrived. Here are some tips on how to prepare for your first visit, and links to download our welcome documents for you to fill out ahead of time, saving you time.
If this is your first visit please be sure to bring with you:
- Your health care insurance coverage may have been discussed before you arrive, but please bring your health insurance ID card.
- A list of all prescription or non-prescription medications you have recently taken or are taking now.
- We have an in-house insurance plan for non-insured patients!
For your convenience, we have our new patient documents here, which you can open, print and sign, to bring in when you have your appointment. (Fill in what you can and we will do the rest.)
